On occasion during Phases 1 and 2, there are a few Cult of Shut'thend that linger from before the Pyramid event was started. Most go away and generally they all go away once the event is triggered.
Would not be a big deal, but is in the area on the sides of the Pyramid where the lower level Hatchling/Juvenile Spawn of Cthylla are located.
Personally I think Cthylla is being sneaky and leaving a few of the Cult of Shut'thend as spies behind! To foil our plans of defending the Pyramid!
